Disseminating best-evidence health-care in Indigenous healthcare settings and programs in Australia.
The aim of this critical review is to describe and critique methodological aspects of interventions targeting reductions in smoking, poor nutrition, alcohol misuse and physical inactivity among Aboriginal Australians. An electronic search of 8 databases and a manual search of reference lists of identified literature reviews and the Australian Indigenous HealthInfoNet bibliography for intervention studies published in peer-reviewed journals from January 1990 to August 2007 were undertaken. For inclusion in the review, alcohol, smoking, nutrition or physical activity needed to be the primary focus of the study and the intervention needed to target Aboriginal Australians. Twenty studies were selected for inclusion in the review. Methodologically, few intervention studies employed randomization or a control group, most omitted important details (e.g. costs), some did not report process measures (e.g. attrition rates), and some did not use validated measures. Two-thirds of interventions were implemented at the community level and employed multiple strategies. There is a clear need for more rigorous evaluations of interventions targeting Aboriginal Australians and to establish the reliability and validity of measures to quantify their effect. There is also clear scope for increased focus on the evaluation of secondary preventive interventions to complement the current concentration of effort in primary prevention interventions. Increased efforts are required on the part of researchers and funding bodies to ensure that intervention research targeting Aboriginal Australians is methodologically rigorous and provides robust evidence, thereby informing healthcare practitioners, policy makers and Aboriginal Australian communities as to what might reasonably constitute best practice in Aboriginal health care settings.

Increasingly, the role of health research in improving the discrepancies in health outcomes between Indigenous and non-Indigenous populations in developed countries is being recognised. Along with this comes the recognition that health research must be conducted in a manner that is culturally appropriate and ethically sound. Two key documents have been produced in Australia, known as The Road Map and The Guidelines, to provide theoretical and philosophical direction to the ethics of Indigenous health research. These documents identify research themes considered critical to improving the health of the nation’s Indigenous peoples. They also provide values that, from an Indigenous perspective, are foundational to an ethical research process. This paper examines these research themes and values within the context of a current longitudinal birth cohort study of Indigenous infants and children in south-west Sydney: the Gudaga Study. Considerable time and effort have been invested in being true to the values stated in these documents: reciprocity; respect; equality; responsibility; survival and protection; and spirit and integrity. We have learnt that it is vital to be true to these values when conducting Indigenous health research—to quite literally “walk the talk”.

This study aimed to understand the problems within the Indigenous health policy process in Australia and how weaknesses in the process impact on policy implementation. Using semi-structured questions, 23 key stakeholders in the policy-making process were interviewed. Three main themes dominated; a need for increased Indigenous involvement in policy formulation at the senior Australian Government level, increased participation of Indigenous community-controlled health organisations in the policy-making process and, most importantly, ensuring that policies have the necessary resources for their implementation. The emergence of these specific themes demonstrated weaknesses in policy process from the formulation stage onward. Tackling these would, according to our informants, significantly enhance the effectiveness of the policy process and contribute to further improvement of the health of Indigenous Australians.
